Form 486 Filing Information

Title of Form: FCC Form 486 — Schools and Libraries Universal Service Receipt of Service Confirmation Form

Purpose of Form: Form 486 notifies the Schools and Libraries Division (SLD) that the Billed Entity and/or the eligible entities that it represents is receiving, is scheduled to receive, or has received service in the relevant funding year from the named service provider(s). Receipt by the SLD of a properly-completed Form 486 triggers the process for the SLD to receive invoices.

Form 486 must be filed to accomplish the following:

To authorize the payment of invoices from the service provider(s)
To indicate approval of Technology Plans, as required
To indicate the status of compliance with the Children’s Internet Protection Act (CIPA) (Pub.L. 106-554).

When to File Form 486: Form 486 cannot be filed before receipt of a Funding Commitment Decision Letter (FCDL) from the SLD.

In general, Form 486 must be postmarked or received no later than 120 days after the Service Start Date featured on the Form 486 or no later than 120 days after the date of the FCDL, whichever is later, in order to receive discounts retroactively to the Service Start Date.

Early filing: Applicants who have received an FCDL and who have confirmed with their service providers that services will start in July of the funding year can file Form 486 before the start of the funding year.

SLD Letter: The SLD will issue a Form 486 Receipt Notification Letter to both the applicant and the service provider(s) upon successful data entry of the Form 486 and the Form 486 Block 4 certifications.
