Problem Resolution

Forms submitted manually to the SLD - including manually submitted certifications - must first meet Minimum Processing Standards. If your manually submitted form or certification fails Minimum Processing Standards, it will be rejected and returned to you. If your manually submitted form or certification meets Minimum Processing Standards but cannot be successfully data entered, it enters a phase of processing called "Problem Resolution."

During Problem Resolution, a representative of the SLD Problem Resolution Team will attempt to make contact with the contact person identified on your form to obtain the information necessary so that data entry can be completed. You may be asked specific questions, and the representative may request additional documentation.

The SLD strongly urges you to have copies of all the forms you submit to the SLD readily available, as well as any documentation you used in order to complete these forms (such as billing information, contracts, or information about entities receiving service). This will aid in your discussion if the Problem Resolution Team contacts you.

Information on deadlines for responding to these Problem Resolution requests for information and/or documentation is contained in the Deadline for Information Requests posted in the Reference Area of this web site.
