EZ Filing Guide for Form 470

SLD has developed a tool to ease the Form 470 application process for certain applicants. This EZ Filing Guide can be used only by applicants who are applying for discounts on basic local and long distance telephone service. No Technology Plan is required for basic local and long distance telephone service.

See the list that follows this EZ Filing Guide to determine if you can use this Guide; you are eligible to use this Guide if you meet the six criteria described there.

If the following statements are true, the applicant can skip seven of the 25 numbered items on the Form 470, some of which have multiple entry fields. In addition, the answers to items concerning technology plans can be answered "No technology plan needed."

1.

The applicant is applying for discounts only on basic local and long distance telephone service.

2.

The applicant’s contact information is the same as the contact person’s contact information.

3.

The applicant does not need to identify a technical contact person.

4.

The applicant does not have restrictions imposed by state or local laws or regulations on bidding procedures.

5.

The applicant does not wish to describe, or does not have, plans to purchase additional services in future years.

6.

The applicant does not include any ineligible participating entities.


If these statements are true, you may use the guide above to fill out your Form 470.

Note: If you file electronically, some of the required items may also be filled in for you automatically.
