Having a Problem Submitting a Question?

If you are having difficulty using the "Submit a Question" system, please call the Client Service Bureau (CSB) at 1-888-203-8100.  We appreciate your effort to utilize our Online Submit A Question Feature.  By providing answers to the questions below, you will assist us to better serve you.

1. Can you describe what happened?
   
2.  What was the last action you performed before the problem occurred?

Are you getting an error message?

  • If you try again, do you get the same error?
  • Can you print a screen shot of the error message or provide the information contained in the message?
4.  Are you trying to submit a question using an existing case number? If so, what is the case number?
   
5.

If you do not have an existing case number, are you trying to submit a topic inquiry or a form inquiry?

  • Which topic or form did you choose?
  • Which question did you choose under this specific topic or form?
6. Were you able to continue after entering your Name and Contact information?
   
7.  Which operating system are you using (e.g., Microsoft Windows XP)?
   
8. Which browser are you using to access the Internet (e.g., Netscape Navigator version 9.0)?
   
9. Do you experience the same error if you use the “Submit a Question” feature from a different computer?
   
10. What was the date and time when your error occurred?
   
11. Is this your first time using the “Submit a Question” feature?

When you have answered these questions, the CSB agent will give you a case number. After you have received your case number, please prepare a System Information Report for CSB as follows:

1. Follow the link below to the System Information Report.
   
2. Using your browser, print a copy of the Report.
   
3.  Write your name, phone number, and CSB case number at the top of the printed Report. If you have a screen shot of the error message you received, write this information at the top of the screen shot and include the screen shot with your Report.
   
4. Fax your Report to CSB at 1-888-276-8736.
