  1. Certain Year 3 Applicants Permitted to Refile Under Special Waiver

    A number of applicants were identified in the FCC Order released on August 11, 2000 (FCC 00-260) which pertained to the Year 3 filing window. These applicants were given a special waiver to refile Forms 470 by September 11, 2000 and properly complete and file Forms 471 by December 11, 2000. The SLD will consider such Forms 471 filed by these applicants as if they were received within the original filing window that closed on January 19, 2000.

  2. Form 498 Introduction (12/13/99)

Who must File Form 498?   All service providers participating in the Schools and Libraries Universal Service Program must file a FCC Form 498, Service Provider Information Form.

What is Form 498?  The Form 498 is used to collect pertinent billing, remittance, and other information from service providers and vendors who participate in the Schools and Libraries Program.   Completion of this form initiates the assignment of a Service Provider Identification Number (SPIN).  All service providers and vendors must submit the completed Form 498 to provide USAC with the necessary information to make direct payments of any invoices submitted for Universal Service Funds.

How is the Form 498 Related to the SPIN?  Telecommunications Service Providers contributing to the Universal Service Fund have already received their SPIN after submitting FCC Form 457 to the Universal Service Administrative Company (USAC).  For other participating vendors, USAC will assign a SPIN when the vendor properly completes and submits Form 498, along with the Letter of Authorization. 

How to Obtain a Copy of Form 498:   If you have not yet been assigned a SPIN and intend to participate in the Schools and Libraries (E-rate) program, file Form 498 with USAC.  The Form 498 must be accompanied with a letter of authorization (see Form 498 instructions for more information).

A copy of Form 498 may be obtained:

  • Via the internet - www.universalservice.org/info/forms.html

  • Send a Request Via Fax - (888) 637-6226.   The fax should include the Company Name, Contact Person's Name, Address, Phone Number, Fax Number, E-mail address, and how Form 498 should be returned (Fax, E-mail, or US Mail).

  • Request Via Phone - (888) 641-8722.   Call Monday through Friday, 8:30 AM - 4:30 PM ET.

Where is the Completed Form 498 Mailed? Form 498 and the Letter of Authorization must be mailed in, not faxed, in order to have a valid signature on file.  Send the properly completed Form 498 and signed Letter of Authorization to:

  1. News for Vendors about Pre-Posting of Forms 470 for Year 3!

Applicants with state or local procurement laws requiring a multi-month competitive bidding process may be filing Form 470 applications RIGHT NOW for Year 3.

Even as the SLD finalizes changes to the Year 3 application process, it is allowing applicants to file Year 2 Form 470 applications for services they are seeking in Year 3. Vendors are encouraged to monitor the Form 470 information submitted daily to the SLD in order to submit bids for service in Year 3 .

Vendors may download daily Form 470 Information from the SLD Web Site at www.sl.universalservice.org/ServiceProvider/470Dwnld_FundYearSelect.asp. If new Form 470 information is listed for a potential customer, vendors may find out more detailed information in the Search Posted Section of the SLD Web Site at www.sl.universalservice.org/Search_FundYear_Select.asp. Be sure to select Funding Year "07/01/1999-06/30/2000) from the drop down menu, and check Block 3, Item 15, to determine if the Form 470 was filed for Year 3 services.

  1. Update of Service Provider Invoice Forms (3/23/99)

If you have submitted a Service Provider Invoice Form (Form 474) more than forty days ago and have not received payment on that invoice and have not heard from the SLD, you can inquire about the status of that invoice by calling (973) 428-7335 Monday through Friday from 8:00 a.m. – 5:00 p.m. EST.

Questions on how to complete the Service Provider Invoice Form (Form 474) should be directed to the SLD help line at (888) 203-8100.

If you have submitted a Billed Entity Applicant Reimbursement (BEAR) Form (Form 472) more than forty days ago and have not received a BEAR notification letter and have not heard from the SLD, you can inquire about the status of that BEAR by calling (973) 428-7338 Monday through Friday from 8:00 a.m. – 5:00 p.m. EST.
