Information About FCC Form 498 and the Service Provider Identification Number (SPIN)

Service providers participating in one or more of the support mechanisms of the Universal Service Administrative Company (USAC) are required to have a Service Provider Identification Number (SPIN).  The primary purpose of Form 498 is to collect contact, remittance and payment information for service providers that receive support from the federal universal service support mechanisms. Manufacturers who are requesting a SPIN only for participation in the Eligible Products Database program do not need to provide any bank remittance information to USAC. Therefore, they may omit lines 102-105 in Block 10 concerning bank remittance information.

These suggestions are provided to manufacturers requesting a SPIN only for participation in the Eligible Products Database:

• Select "Original Application for SPIN" if you do not have an existing SPIN or are requesting a new SPIN.
• If you are aware that your company is a contributor to the Universal Service Fund, please provide your 499 Filer ID.  If you do not know the 499 Filer ID for your company, please refer to your company's regulatory department or you may search the FCC's 499 Filer database.
• Complete Blocks 1, 2, and 3.
• You need not complete Blocks 4 through 9.
• Complete Block 10, but you may omit lines 102-105 concerning remittance information.
• Complete Block 11.
• You need not complete Block 12.
• Complete Block 13.  You may select "Non-traditional Provider" as your business code.
• Complete Block 14.

On average, a Form 498 will take approximately two weeks to process.
