Instructions for Filing FCC Form 470 to Support Additional Year 2 Funding Requests

While it is unlikely that an applicant can file a Form 470 (Description of Services Requested and Certification Form) now for Year 2 support, you should follow the instructions below if you wish to try.

Form 470 applicants must use a "Year 3" FCC Form 470 (dated September 1999), available on the SLD web site, on-line and in a format to be downloaded for manual filing. Applicants should indicate in Items 8, 9, or 10, as appropriate, that these requests are for Year 2 services (rendered July 1, 1999 to June 30, 2000). Online filers will be required to enter Funding Year 3 dates (2000-2001) in Block 1, Item 2 for system purposes even though the funding request will be for Year 2 services as identified in Items 8, 9, or 10. Manual filers should indicate 1999-2000 in Block 1, Item 2.

Manual filers who use standard U.S. Postal Service mail should send their Form 470 to:

Certification forms signed by electronic filers must be sent to one of the above addresses.
